Our GP practice offers comprehensive Chronic Disease Management (CDM) services to eligible patients, as part of the Structured CDM Programme

What is Chronic Disease Management (CDM) Services?

The HSE's Structured Chronic Disease Management (CDM) Programme aims to improve the health and well-being of patients living with specific chronic conditions through ongoing monitoring and support.

This programme is available to individuals aged 18 and over who have a medical card, GP visit card, or Health Amendment Act card and have been diagnosed with one or more specified chronic diseases.

Key features include regular reviews, where eligible patients receive two visits every 12 months with their GP or practice nurse, along with personalised care plans tailored to their specific needs.

What is a Chronic Disease and What's Covered?

A chronic disease is a long-term health condition that requires ongoing treatment and management. These conditions can significantly impact your quality of life and often develop over time due to various factors, including genetics, lifestyle, and environmental influences.

Conditions Covered by the CDM Programme

The HSE's CDM Programme covers the following specific chronic diseases:

  1. Type 2 Diabetes

  2. Asthma

  3. Chronic Obstructive Pulmonary Disease (COPD), also known as chronic bronchitis or emphysema

  4. Cardiovascular diseases, including:

    • Heart Failure

    • Heart Attack (Angina)

    • Stroke

    • Irregular Heartbeat (Atrial Fibrillation)

Why is Regular Health Monitoring Important?
